Primaris offers free assistance to healthcare providers based in Missouri's Critical Access Hospitals in the area of quality reporting. Our assistance can help providers improve their rankings on hospitalcompare.com and ultimately, treat patients more effectively.

This patient-centered project, the Effective Transitions of Care Project for Critical Access Hospitals, will work in parallel to Inpatient Prospective System hospitals working toward similar improvements in care.

The project aims to assist Critical Access Hospitals in fully informing patients about the care they receive, equipping them for follow-up care. Furthermore, the project hopes to foster improvements in patient safety and more effective transitions between care settings. Long-term goals include reducing the duplication of services, helping patients stay healthy after discharge and avoiding unnecessary hospital re-admissions.

Here's what Primaris can do:

  • Coordinate one-on-one technical assistance regarding performance improvement, measurement and reporting.
  • Provide access to a network of experts and hospitals working to improve the same aspects of patient safety.
  • Offer comparison to peer Critical Access Hospitals.
  • Ensure bragging rights: providers can publicly report progress and take credit for it.

Specifically, providers at Critical Access Hospitals should report CMS' Outpatient Emergency-Department Throughput Measures, specified in the Hospital Outpatient Quality Reporting Specifications Manual.
