primaris flame Nursing Homes in Need

Primaris provides a special, voluntary “Nursing Homes In Need” Program for Missouri nursing homes designated by the Centers for Medicare & Medicaid Services as “Special Focus Facilities” (SFF).

Medicare describes SFFs as facilities that have:

  • More problems than other nursing homes (about twice the average number of survey deficiencies),
  • More serious problems than most other nursing homes (including harm or injury experienced by residents), and
  • A pattern of serious problems that has persisted over a long period of time (as measured over the three years before the date the nursing home was first put on the SFF list).

SFF designations are based on State Survey Team inspections. Most nursing homes have some deficiencies, with the average being 6-7 deficiencies per survey.

Primaris offers free consultation to find the underlying causes of quality issues and work to help nursing homes graduate out of the Special Focus Facility program.

SFF are also encouraged to utilize free resources available through our Resource Catalog.
