American Association of Kidney Patients - resources to help CKD patients and their families to better understand their condition, adjust more readily to their circumstances and assume more normal, productive lives in their communities

- Interactive educational materials on the website, including Q&A's, chat room, and live "Kidney Beginnings" programs
Kidney School - free, web-based information for patients with CKD Stages 3-4
- 16 modules on all aspects of disease process and management
Missouri Kidney Program - state funded organization offering assistance to Missouri residents with kidney disease
- Patient Education Program (PEP): free for patients with CKD Stage 3 or 4
- Financial assistance with medications and immunosuppressive therapy
Fistula First Breakthrough Initiative - resources to promote AV fistulas as the preferred access for suitable hemodialysis patients

Primaris - the CKD web page will continue to provide the resources we have used during this project. The current resources will remain available for download only. However, we will no longer be mailing out printed tools. Some of the CKD monthly "Tips" are archived here as well.
